Our North County clinic offers a space that's easy to navigate, gentle on the eyes and comfortable for all. Everything from the colors of the walls to the waiting room furniture has been chosen for the benefit of our older patient population.

Our clinic offers older patients routine services they're likely used to receiving elsewhere, including:

  • Medicare annual wellness check-ups
  • General medical evaluations

In addition, we can also help patients treat and manage a number of conditions, such as:

Mercy Prime 65+ FAQs

Mercy 65 Prime+ is the new primary care practice for Medicare beneficiaries.  It’s a primary care practice specially designed to meet the unique needs of Medicare patients. 

  • As a 65 Prime+ patient, you’ll get extended visit times to make sure your concerns can be fully addressed without you or your doctor feeling hurried. 
  • You’ll be seen more frequently to keep chronic conditions like high blood pressure, diabetes and heart disease under control. 
  • Our primary care physicians for senior patients are intensely focused on helping you meet your individual wellness goals and identifying problems early before they can affect your health.
  • You’ll be able to get a routine appointment within a week. 
  • For urgent concerns or acute conditions, you’ll be scheduled within 1 business day.  
  • The foundation of the care team is a Mercy primary care physician solely dedicated to caring for Medicare patients. 
  • Each physician is supported by a registered nurse and a care navigator. 
  • The nurse provides clinical support to patients during clinic visits and care between visits. 
  • The care navigator helps coordinate all other aspects of the patient’s care.

The care navigator helps with all care needs during and outside of visits. This team member:

  • Coordinates referrals to specialists, therapists or other providers
  • Sets up tests or procedures
  • Obtains prescriptions and medical equipment
  • Makes sure that all preventive care like immunizations, lab tests, and cancer screenings are obtained
  • Seamlessly coordinates care when significant events occur, including emergency room or hospital visits
  • Our practices provide complete senior-focused primary care including all wellness care, chronic condition management and acute or urgent care. 
  • Lab services are readily available in or near the practice. 
  • Diagnostic testing such as imaging or cardiac testing happen at existing Mercy facilities. 
  • Patients with a Mercy specialist or other specialist will continue their care with them.

No; geriatrics typically involves a one-time or occasional consultation visit for older adults.  Mercy 65 Prime+ provides continuous primary care for our patients.

Our physicians may be trained in either internal medicine or family medicine, but all our physicians are experienced in and passionate about caring for older adults.
