  • In addition to the medical history form, please bring a photo ID, your current insurance card(s), applicable co-pay and all medications that you are currently taking on a routine basis. This includes prescription and over-the-counter medications.
  • Please try to arrive 10 minutes before your appointment time for check-in.
  • Please note, patients under the age of 18 must be accompanied by a parent or guardian.
