A patient may be a good candidate for referral to admission into our inpatient rehabilitation program if they have a decrease in their ability to function in a normal environment due to an acute injury, impairment, or illness.
The referral could come from any of a number of sources, including:
Acute care hospitals
Long-term acute care hospitals
Skilled nursing facilities
Assisted living facilities
Home health agencies
Emergency rooms
Primary care physicians
Family or friends
The admissions process begins with a referral being called into our admissions coordinator. Once a referral has been made to our facility, a clinical liaison will be sent to complete a clinical evaluation to determine medical stability and appropriateness for admission. Our admissions department will then work with the referral source, insurance company, the physician and the discharge planner to coordinate transfer to our facility.
