Preparing for a Stroke Patient Transfer:
Gather all pertinent information the receiving providers will need in the ER, such as patient name, DOB, presenting symptoms, blood pressure, heart rate, tPA start time and dosage, etc.
Referring physician should contact Mercy Transfer Center for a physician to physician report. 417.820.2022
Ensure patient is level one, acute and 1:1 with a nurse. Maintain Q15 vital signs and neuro checks.
Call dispatch to arrange EMS transportation as a PRIORITY ONE (STAT).
Print medical records, including CDs if advanced imaging was performed. Complete EMS and COBRA forms.

Please note that patients treated with IV tPA should leave your facility within 90 minutes of arrival. Patient NOT treated with IV tPA should leave your facility within 60 minutes of arrival.
