Mercy Hospital Care at Home (HCaH) is a service designed to offer hospital-level care in the comfort of your home. This service allows you to heal and recover where you feel most comfortable, surrounded by your family and friends.

HCaH can help with a variety of conditions sometimes requiring hospitalization like pneumonia, COPD, diabetes and heart disease. Care coordinators will work with you and your family to make sure you have all the support you need around the clock.

Hospital-level care at home has been studied and shown to be as safe as traditional care in a hospital. We're excited to provide this opportunity to bring care to people where they need it most. 

How Mercy Hospital Care at Home Works  

Our care team will assess your status while you're in the hospital to see if you're a candidate for the service. If you're cleared to participate, we'll immediately get to work transferring you from the hospital to your home.

A support person will be with you every step of the way, from gathering the necessary medical equipment to transporting you home safely. All your medications, meals and medical supplies will be provided.

Once home, you'll see your Mercy doctor every day, either in person or virtually. If you don't have a home computer or smartphone for virtual visits, don't worry. We'll provide you with a service tablet free of charge and show you how to use it. You'll have 24/7 access to your care team for peace of mind at all times.

Therapy services, care management and clinical nutrition support will be provided as needed. Typically, patients may spend anywhere from two to five days in the HCaH service. Your primary care team will follow up with you regularly once you're discharged. 

Mercy Hospital Care at Home FAQs

Mercy Hospital Care at Home (HCaH) provides advanced care at home with two daily visits from a registered nurse, one daily virtual or in-person visit from a Mercy physician or advanced practitioner and specialty care as needed. You'll also have access to a patient care technician as needed for personal care, assistance with meal prep and light housekeeping. HCaH also connects you to other support services, such as physical therapy, care management, nutrition consultations and more. 

For Medicare beneficiaries, there's a special waiver service that covers Mercy HCaH costs with the same coverage as sudden hospital care. If you're not a Medicare beneficiary, check with your insurance provider or our HCaH team about coverage. 

You'll receive one visit from a Mercy doctor or advanced practitioner daily, either in-person or virtually. You'll also receive two visits daily from a registered nurse. Your care team is available by phone 24/7. You can reach them with any questions or concerns at 844-735-4710.

Typically, you'll be admitted to Mercy HCaH for two to five days, based on your condition or clinical need. Once discharged, you'll be enrolled in Mercy Care Connect and instructed on follow-up care with your Mercy primary care team.
