Mercy Care Connect

Paying close attention to your symptoms helps you and your care team better manage your condition and keep you healthy. With the Mercy Care Connect (MCC) messaging service, you can easily record your symptoms by phone (either text or automated call) and have them go directly to members of your Mercy care team.

MCC is designed to provide patients with additional support in between appointments with their PCP and other providers. There are a few different MCC modules that a patient could be enrolled in:

  • Diabetes
  • Heart Failure
  • COPD
  • Hypertension
  • Asthma
  • Medication reminders
  • Post-hospitalization support (lasts 30-days following an ED visit or hospital stay)
  • General support module (overall health status check ins)

If you are eligible for enrollment you’ll receive a phone call from a team member, who will provide information about the program and help you enroll if you choose to participate. Once enrolled, we’ll send messages to your phone asking about your current health and changes in symptoms.

  • You can receive either text messages or phone calls.
  • For smartphones or other mobile phones, text messages or calls can be sent. For landline phones, only calls can be sent.
  • You’ll respond by returning text messages or by pressing numbers on your phone.
